The Warheads made it to Hell, but Rathcoole was apprehended just after that, reducing Blackheart's influence slightly. A battle against demons happened, and Perez was killed in combat, which disrupted Blackheart's plans as he needed seven humans to wield the sword.
Blackheart yelled that he had been aiding the Warheads this past year to keep them alive for his own plot, and now he planned to kill them. The mortals grabbed the sword designed to kill Mephisto and threatened Blackheart with it, but Mephisto revealed himself and banished the humans back to Earth. Mephisto then grabbed Blackheart, shrank him down, and consumed him, returning him to imprisonment. Blackheart powered up a group of humans including Vana and Hank , enhancing them with his demonic energies, and calling them his Corrupt.

Mephisto took the form of a bird and taunted Blackheart, who raged at his father's injustices and planned to dethrone him. The Corrupt brought Ghost Rider and Lucy to Blackheart, and he planned to reign with the innocent girl at his side. Punisher and Wolverine closed in and found Blackheart, his power depleted, with a knife to Crumm's throat and Ghost Rider strung up on the ceiling.
Blackheart tried corrupting the heroes' memories, but Wolverine broke free and the heroes overwhelmed Blackheart, who cried for mercy and asked for their help in killing Mephisto. As Blackheart argued that he would be a more humane ruler of Hell, the heroes defeated him with chains, claws, and bullets.
As he lay weak, Blackheart was shocked when Crumm knelt over him and forgave him. He returned to Hell with some of Crumm's blood on a knife. Ignoring Mephisto's promises of power and unity, Blackheart killed his father with the power of the innocent blood and claimed his position as the king of Hell.
Mephisto claimed the throne again before long and imprisoned Blackheart again. Ghost Riders: Crossroads 1 - Blackheart recruited the Scarecrow forcibly, then kidnapped Dan Ketch from a cemetery, stopping him from transforming into Ghost Rider. Blackheart then wore a suit and sought out John Blaze in a bar. After mentally manipulating the patrons to not see him, Blackheart convinced Blaze to put down his Hellfire shotgun then explained that he was seeking to expand his Hell territory to Earth, but he needed to speak to Zarathos to do it.
Blackheart taunted Ketch with the souls of Roxanne Simpson and Barbara Ketch, and promised to let him take someone with him if he escaped Hell, but he made the same promise to Scarecrow and told them both only one could win.
Blaze as Ghost Rider went to a jail and confronted prisoners, as Blackheart taunted the Rider into consuming the souls of the killers there, including serial killer Mikey. Blackheart brought Blaze and Ketch together and taunted the men, tempting them with impossible choices, but they outwitted Blackheart when they chose to keep their Ghost Rider powers themselves.
Blackheart cast them out of Hell, keeping Barbara and Roxanne in the realm among his demons. Ghost Rider III 76 - Blackheart sought out the defeated villain Snowblind and manifested himself, changing the villain's fortune to see what he might do with his success. As Skritch looked on, Blackheart prepared the demonic vessel for Black Rose, his new consort. Skritch introduced the Furies Dark Lady, Ember, Lady Ash , who demanded to see Mephisto, and they demanded human forms with which to attack Ghost Rider, a treaty they had previously formed with Mephisto.
Blackheart agreed to help the women. Blackheart taunted Blaze with news of his missing children, then disappeared with Black Rose before the police could close in. Ghost Rider III 86 - In Hell, Blackheart walked down the Flesh Corridor, full of human souls and faces in conflict, and he returned the Scarecrow there to be tortured for his failure, and he put Black Rose in charge of the torture. He resurrected her, with her willingness, as the goddess of vengeance, Pao Fu. Ghost Rider III 89 - Blackheart transformed a recently killed dog, Chio, and his owner, into the monstrous demonic Doghead, and allowed him to attack a few humans before drawing him down to Hell.
He sent his operatives against Mephisto. Blackheart tortured a woman's spirit to taunt Noble. Blackheart told Kale he wanted to expand his powers across Hell, and that would require him to renegotiate Mephisto's treaties with the other realms. He offered to make Pao Fu the wife of Kale if Kale agreed to lead Blackheart's operatives, and offered to reunite Kale's soul with his mortal body, which had been preserved in Hell for centuries.
Kale agreed to serve Blackheart if he was given both Black Rose and Pao Fu as his consorts, and Blackheart agreed, returning the heart to Kale, and reuniting soul with body. Noble Kale stood in a new form, ready to lead the others. Blackheart gave Black Rose a weapon made from Zarathos' rib, one capable of killing Kale, and he told her to use it after the wedding but before the consummation.
Kale attacked Blackheart directly, then Vengeance joined the fight and Blackheart was overwhelmed. Kale seemingly destroyed Blackheart in a blast of power, then Kale was proclaimed the new lord of Hell. Blackheart exposed the worst fears of the heroes, but Warpath was able to triumph against Stryfe and guide his friends out of Hell. Blackheart planned to control the entire planet, proving himself Mephisto's better. After they imprisoned Daimon Hellstrom to avoid detection, they began gathering their power.
Fantastic Four Annual - Blackheart incapacitated Moy with a soul-stealing spell, then he initiated a long plot against Moy's allies, the Fantastic Four. At the Hellfire Club, Selene summoned Blackheart, and he manifested as a giant demon, transforming the mortal guests into demons themselves. The Fantastic Four, Mechamage, and Margali Szardos attacked, defeating the transformed demons, then turning the attack on Blackheart himself, even as Blackheart transformed the Club into part of his realm.
Mechamage managed to free Hellstorm, who joined the attack against Blackheart, and a spell was cast that trapped Blackheart and Selene in the Hellfire Club.
X-Force I 98 - Blackheart remained trapped in the Hellfire Club, projecting himself outward astrally as he could, as Selene trapped X-Force Warpath, Meltdown, Bedlam, Cannonball, Sunspot in the building in their own personal nightmares.
Blackheart kept Moonstar personally captive for a time, taunting her as he reviewed the lists of souls he was to collect, but Hela threatened war unless he let her go, and Blackheart reluctantly agreed. Johnny turns into Ghost Rider and attempts to perform the Penance Stare on him but fails due to Blackheart having "no soul to burn". Blackheart then kidnaps Roxanne and tells Johnny to bring the contract of San Venganza in exchange for her. Johnny goes to San Venganza with the contract but instead of handing it over, he attempts to kill Blackheart.
During the battle with Ghost Rider, Blackheart manages to subdue him long enough to get the contract and summon the souls to absorb them. With the 1, souls within him, he turns into Legion. Ghost Rider realizes that he can now preform the Penance Stare on Blackheart due to him being filled with the condemned souls. Which he uses to take down Blackheart once and for all.
After doing so, Mephistopheles arrives and presumably takes his son's catatonic body to the back to Hell.
